The Department requires certain taxpayers to confirm their identity in an effort to combat tax fraud and safeguard taxpayer dollars. Taxpayers are selected for identity verification using data analysis of information on the tax return and other factors.

Note: Tax fraud schemes are ever-changing. Thus, you may be asked to confirm your identity on a tax year by tax year basis.

The identity confirmation quiz letter can be reissued to the exact address provided on the return when filed. To have this letter reissued, please call 1-800-282-1780 (1-800-750-0750) for persons who use text telephones (TTYs) or adaptive telephone equipment) from 8:00 am to 5:00 pm, Monday through Friday excluding holidays.

Although the reissued letter will show the current date, it does not extend the original timeframe for taking the quiz.

You received an identity confirmation quiz letter because an Ohio income tax return was filed using your SSN. You should check with your spouse or tax preparer to ensure an Ohio return was not legitimately filed on your behalf.

If you are sure that the return was not filed by you, your spouse, or your tax preparer, you may be a victim of tax-related identity theft. To confirm that the return is suspect, please call us at 1-855-855-7579 (1-800-750-0750) for persons who use text telephones (TTYs) or adaptive telephone equipment). Representatives are available 8:00 am to 5:00 pm, Monday through Friday excluding holidays.

If you determine that the return was legitimately filed on your behalf, you should take the identity confirmation quiz. See the FAQ ”How do I take the identity confirmation quiz?” for more information.

You can only take an identity confirmation quiz if you received a quiz letter from the Department of Taxation. Your letter will provide you with the timeframe allotted to take the quiz. Processing of your return does not begin until your quiz is passed. To take the identity confirmation quiz:

  1. Go to the Department’s Identity Confirmation page;
  2. Select the following option:Green Checkbox
  3. Check the “I Agree” box (see below) and enter the Reference Number and Authorization Code shown on your letter;
  4. Enter your “State of Ohio Refund Dollar Amount” as a whole number (no decimal places);
  5. Correctly respond to the Verification Question;
  6. Click the Next button at the lower right portion of the screen.

Note: By checking the “I Agree” box, you authorize the Ohio Department of Taxation, under the FCRA (Fair Credit Reporting Act), to obtain information from your personal credit profile for the sole purpose of confirming your identity. See 15 U.S.C. §1681. Taking the identity confirmation quiz has no effect on your credit score.

Once you have accessed the quiz, you will have 5 minutes to answer four multiple choice questions. If technical difficulties prevent you from completing the quiz in the time allotted, it will count as a failed attempt.

After you have selected an answer for all four questions, click the Submit button. You must answer three of the four questions correctly to pass the quiz.

Refer to the tax return identified on the identity confirmation letter (either Ohio IT 1040 or Ohio SD 100). The amount will be listed directly to the right of “YOUR REFUND” on the return. Enter the amount in whole dollars (no decimal places). Do not use your federal refund amount.

Upon passing the quiz, your return will begin processing. You can begin checking your refund status two business days after passing the quiz. You will be notified if additional information is needed to complete the processing of your return.

You can take the quiz a total of three times online. If you fail your first or second online attempt, you will be given another chance to pass the quiz online. If you fail all three online attempts, you can take the quiz an additional two times by calling the Department of Taxation at 1-855-855-7579 (1-800-750-0750 for persons who use text telephones (TTYs) or adaptive telephone equipment) from 8:00 am to 5:00 pm, Monday through Friday excluding holidays.

Note: Because the Department does not store the information used for your quiz, and it is immediately discarded upon the quiz’s completion, the Department does not know which questions you answered correctly or incorrectly.

If you are unable to access the quiz, it may be that:

  • There is not enough information available to present you with an identity confirmation quiz;
  • The time period to take your identity confirmation quiz has expired; OR
  • Steps have been taken to protect your identity (i.e. credit freeze).

The information used for your identity confirmation quiz comes from a combination of public and commercial data sources. The data consists of current and historical information about you. The information used complies with all applicable privacy laws.

Your information is not accessed until you begin the quiz, and is immediately discarded upon the quiz’s completion. This information will never be stored or shared for any purpose.
