Business Address Update Form

Complete each section below to update your business tax location and/or mailing address. A current address will ensure that you receive important mailings pertaining to your business account(s).

If you are a third-party taxpayer representative, a completed TBOR-1 must be on file or your request will not be processed. Form TBOR-1 can be found on the Department's forms page.

Section 2 of 4: Select Tax Type
    1. *If you have an existing fixed place of business and are moving to a new location not within the same county, you must cancel your vendor’s license and obtain a new vendor’s license by applying online at Ohio Business Gateway or at the county auditor where your new place of business will be located.

    1. *If you are changing your fixed place of business you will be subject to a $25.00 license fee. Please contact the Excise tax unit at 855-466-3921, option 3, for instructions on remitting your fee payment.
