If you travel interstate with a three-axle truck, a tractor, or a two-axle vehicle where the registered GVW is greater than 26,000 pounds, you will need an IFTA permit.

You can obtain an application from the forms page on our Web site.  Once completed, you can then fax or mail it to the IFTA Permit Unit.  The fax number is (614) 728-8085.  The mailing address is Ohio Department of Taxation, IFTA Permit Unit, P.O. Box 530, Columbus, Ohio 43216-0530.

You can also request, on the same form, a temporary operating authority to keep in your cab until your decal is mailed to you.  Allow five days to receive your decal.
