Please list all employment and educational information on the State of Ohio GEN-4268 application form. You can complete this form on-line or print, complete, and electronically forward it to our office. In addition to the State of Ohio GEN-4268 application form, below are links to print four other forms required by our agency. These forms cannot be completed and submitted on-line. Forms Forms 2 and 3 listed above should be printed by you, completed, NOTARIZED where required, and returned to our office with forms 1 and 2 at the address listed below:
Forms returned to our office in person by the applicant can be notarized free of charge.
