PLEASE COMPLETE THE FOLLOWING FOUR FORMS IF APPLYING FOR EMPLOYMENT:

NOTE:  Ohio Department of Taxation does not utilize cover letters or resumes in the processing of applications. Please list all employment and educational information on the State of Ohio GEN-4268 application form. You can complete this form on-line or print, complete, and electronically forward it to our office.  In addition to the State of Ohio GEN-4268 application form, below are links to print four other forms required by our agency.  These forms cannot be completed and submitted on-line.

Forms 2 and 3 listed above should be printed by you, completed, NOTARIZED where required, and returned to our office with forms 1 and 2 at the address listed below:

Forms returned to our office in person by the applicant can be notarized free of charge.  If you have any questions or would like an application package to be mailed to you, please call our Human Resources office at (614) 466-3020.
